CHAIN

A succession of incidents, occurring at various intervals along the time track, that are related to one another by some similarity of either subject, general location, people or perception. Such a succession of similar incidents may span a brief period or a very long period of time.

Usage

In auditing, a chain is a series of related incidents on the time track, connected by similar subject matter, location, people, or perceptions. The auditor traces the chain back through its incidents to find and address the earliest one.

Why It Matters

Chains explain why a single type of upset or reaction can persist over a long period — each new incident adds to the chain and reinforces the original charge. Auditing a chain to its basic incident resolves the entire series.
